Your Eligibility
In
order to enroll in and contribute to an HSA, you must
first confirm that you are not covered by any health plan
other than an HDHP, that you are not enrolled in Medicare
and that you are not able to be claimed as a dependent
on another person's tax return. Click

How
to Enroll in an HSA
Electronic
enrollment is currently under development for this benefit,
but is not yet operational on
our
website. However, forms to enroll are available
through your employer's Human Resources Department.
Please continue enrolling only if you have reviewed the
HSA page as directed above and have confirmed that you
are eligible to participate in an HSA. If you have
determined that you are not eligible, then you may not
enroll in this benefit.
Forms
Required for Enrollment
Please
contact your employer's Human Resources Department to
obtain the forms required for enrollment. In addition,
completed forms should be processed through Human Resources
before submitting them to Chard Snyder.
