What if I fail to submit receipts to verify a charge?

If your claim needs to be verified, you will receive an email from Chard Snyder requesting an itemized receipt, insurance explanation of benefits (EOB) or an itemized statement from your doctor or provider. If we do not receive a response, a second notice will be sent 30 days after the first. If we do not receive a response to the second notice, a final notice will be sent 30 days after the second notice. Your Chard Snyder benefit card will be suspended the day the final notice is sent. In order to reinstate your card, you must provide the required records or repay the amount in question.

The first letter is sent out to notify you that we need additional information once the system has reviewed all data feeds. This can occur in as little as one day, but can take as long as 120 days.
